**Hours**:Full time - 37.5 hours per week over 5 days
**Salary**:£10.71 / hour
**Job Summary**
The Care Co-ordinator will be expected to assist clinical personnel in the care of practice patients to include treatment, preventative care, screening and patient education. The post will also include some administrative duties. The Care Co-ordinator will act as a focal point of communication between patients, and other clinical and non-clinical members of the practice.
**Responsibilities**:
To provide and arrange a holistic approach to patients care, including QOF recall, national screening programmes, chronic disease management and signposting to services such as social prescribing and health and well being coaches.

- To deal with new patient enquiries and registrations and signposting to relevant practice services and appointments
- Scheduling patient treatments or other procedures (e.g. Minor Surgery, Coils) with follow up on care and outcomes
- Ensuring records are available and complete prior to patient visits
- Ensuring documents, forms and consents are completed prior to and after treatments
- Co-ordination of reminder procedures and QOF annual recall system
- To be a first point of contact for patients requiring general information relating to the practice: procedures, registration finance etc.
- Providing lifestyle advice to patients, making any necessary referrals within the practice
- Participating in the administrative and professional responsibilities of the practice team
- Ensuring accurate and legible notes of all consultations and treatments are recorded in patients’ notes
- Ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice
- Assisting with online services e.g. patient access, doctor link, myGP
- Attending and participating in practice meetings as required
- Responsibility for coordinating the administration of the PCN DES contract and to work with other PCN staff where necessary
- Being the first point of contact for newly pregnant women who wish to access support and pregnancy vaccines.
- Assisting in the organisation and delivery of the practice’s annual Flu vaccination programme and the Covid-19 Vaccination programme.

- Maths and English GCSE Grade A-C (4-9) or equivalent
- Health and Social Care NVQ / Management NVQ or equivalent
**Experience**
- Experience of working withing primary care
- Experience of working within an administrative department
- Experience of working with QOF and/or associated quality contracts
- Experience of working in general practice
**Skills, Abilities and Knowledge**
- An ability to use own judgement and work independently
- Ability to work without direct supervision and determine own workload priorities
- Ability to coordinate across multiple departments / members of a team
- Excellent organisational skills
- Knowledge of local pathways including community services, secondary care and voluntary secto
