**Job Description: Team Leader**
**Location**: Client
**Reports To**: Clinical Manager (or Clinical Member on Duty)

The pay rate for this enhanced role is £18 per hour when working in this Capacity according to the rota.

**Objective**:
The Team Leader is responsible for providing personalised and comprehensive care to clients, working closely with the Clinical Manager (or Clinical Member on Duty) to address any changes or needs in the client’s care plan. This role ensures a seamless and responsive care experience while maintaining high standards of care delivery.

**Key Responsibilities**:
**1. Client Care**:

- Serve as the primary carer for your assigned client, ensuring all company processes and procedures are followed in liaison with the Clinical Manager.
- Deliver hands-on care and ensure all aspects of the care plan are executed effectively.
- Regularly assess client needs and collaborate with the Clinical Manager to update personalised care plans.
- Ensure the adequate supply of medications and other necessary supplies for clients.
- Perform health and safety checks, audits, and implement emergency procedures as needed, in coordination with the Clinical Manager.
- Monitor care quality and promptly report any discrepancies to the Clinical Manager (or Clinical Member on Duty) and work together to resolve them.

**2. Team Leadership and Development**:

- Coordinate the activities of the carer team, fostering a collaborative environment.
- Ensure new staff are effectively inducted, welcomed into the team, and supported in learning the client’s routine and specific needs, in liaison with the Clinical Manager.
- Collaborate with the Clinical Manager to organise meet and greets and shadow shifts.
- Work with the client and the Clinical Manager to manage expectations and clarify roles for new care workers.
- Support the Clinical Manager in maintaining a stable and consistent team for the client, ensuring fair shift allocations.

**3. Operational Duties**:

- Assist in investigating incidents and complaints, reporting findings to the Clinical Manager (or Clinical Member on Duty).
- Maintain communication with the office team and the client to enable effective team management and rota planning, in consultation with the Clinical Manager.
- Act as a direct point of contact for the Paragon Home Healthcare office and field teams, ensuring cohesive care delivery in alignment with the Clinical Manager’s guidance.

**4. Client and Family Interaction**:

- Collect and provide feedback from clients and families to the Clinical Manager or Coordination Lead.
- Document all care activities and ensure compliance with data protection regulations, coordinating with the Clinical Manager to ensure all records are accurate.

**Criteria for Role Assignment**:

- The Team Leader role is established based on client needs and is determined by the client and Clinical Manager, reflecting the personalised nature of the position.

**Working Conditions**:

- This role is based on your specific rota on PP and as required by the client, with flexibility needed to meet client and team needs.

**Person Specification**:
**Qualifications and Experience**:

- Extensive experience in complex homecare, with a strong understanding of client care needs and planning.
- Knowledge of health and safety, regulations and emergency procedures.
- Strong communication and interpersonal skills, with the ability to work closely with the carer team, Clinical Manager and other stakeholders.
- Good organisational skills and a focus on maintaining high-quality care standards.
- Proficiency in care management software (ACP and Ping)
- Full UK driving licence and access to a vehicle (Desirable).
