Carer Specialist Worker

An exciting opportunity has arisen for a Carers Specialist Worker within the Adult Social Care Community Team.
This dynamic and busy team is responsible for the continued assessment, review and support of people who have a long term condition - aged 18+, including older people.
As a Carer Specialist Worker you will be part of an experiences and supportive team and have the opportunity to work with people from a wide variety of backgrounds in their own homes and various community settings.
Tasks include undertaking Strength Based Carer Assessments and Reviews, in line with Care Act legislation and the setting up Carers Cash Payments. You will be working with carers to look at options for meeting identified outcomes within community and voluntary resources as well as more formal resources.
The ability to develop working partnerships with carers to tease out and address difficult issues. To recognise and understand the challenges and emotions that many cares will face culturally, socially or within their own network. Have the ability to explore creative options for customers.
The post also requires you to have I.T skills, be highly organised and that you are able to thrive and work effectively within this very fast moving environment.
The post is classified as a hybrid working one which means that you may be working part of the week from West Offices in York and rest of the week remote working from home.

At the moment due to the impact of the ongoing COVID-19 pandemic, some of our staff are working remotely from home but a return to a combination of working remotely and physically from the Council’s offices is anticipated in the near future.
As a new starter, your contractual location will be the designated council office for your team. Your working arrangements will vary according to your service and role, and the current Government guidance. For many roles, there will be more opportunities in the future for remote and hybrid working. However, this comes with the expectation that, as workplaces open up, there is likely to be an ongoing and occasional requirement to attend the workplace.
