To identify and assess any children and families in need, and to work in partnership with Primary Care, Children's Centres, Children's Services, Voluntary Sector and other appropriate agencies, to deliver integrated programmes of care within the local area.

To provide a continuum of support according to need, improving outcomes for children by reducing health inequalities. To support the Early Help agenda to ensure early intervention.

To be aware of the Key Performance Indicators for the Children's Division and focus service delivery towards their achievement.

To be responsible for the quality and management of the RiO electronic patient record and data reporting system within the team.

To be the principal point of contact for allocated GP practices and Children's Centres within the area. To develop and deliver integrated working arrangements with these services that support healthy outcomes for children.

To identify and assess children 0-4 with complex health needs, offering an enhanced integrated service within the multi-agency arena, enhancing equality and opportunity for them.
