**Job Summary**

As a Children & Young Peoples Champion the post holder will work as part of a multidisciplinary team, within the Integrated Neighbourhood Teams, to develop and contribute to improving the experience of Children & Young People (CYP) requiring mental health & wellbeing support. The role is part of an overall drive to support CYP to get the right support at the right place first time and to provide early intervention to reduce the risk of escalation or crisis.

The role is new, likely to evolve and develop over time with the aim of collecting quantitative and qualitative data to evidence the impact, leading to more substantive funding. The expectation is that the post holder will develop robust relationships and knowledge of CYP services across Hillingdon and the Integrated Neighbourhood Teams, to act as a resource for primary care and the wider system.

**Primary Responsibilities**

**Collaborative Working Relationships**
- Foster and maintain strong links with all services across the PCN and neighbouring networks.
- Explores the potential for collaborative working and takes opportunities to initiate and sustain such relationships.
- Establish robust relationships with CYP services across Hillingdon, acting as a resource for primary care and the wider system.
- Demonstrates use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations e.g. ICBs, Trusts).
- Work closely with Integrated Neighbourhood Teams to ensure a cohesive approach to CYP mental health and wellbeing support.
- Create and execute strategies to enhance the mental health and wellbeing of CYP, ensuring they receive timely and appropriate interventions.
- Act as a resource for primary care and schools, offering guidance and support to ensure the best outcomes for CYP.
- Can recognise personal limitations and refer to more appropriate colleague(s) when necessary.
- Continuously evaluate and improve the role, adapting to the evolving needs of CYP and the services provided.

Liaises with other stakeholders as needed for the collective benefit of patients including but not limited to:

- Patients and their representatives.
- GP, nurses and other practice staff.
- CYP services including Local Authority Services, The Hillingdon Hospital and 3rd sector services.
- Other healthcare professionals including community pharmacists, pharmacy technicians, social prescribers, first contact physiotherapists, physicians associate and paramedics.
- Community pharmacists and support staff
- Other members of the medicines management (MM) team including pharmacists, technicians and dieticians
- Community nurses and other allied health professionals e.g. School Nurses and Health Visitors
- CNWL Children and adolescent mental health services and Child Development Services
- Hospital staff
- Community nurses and other allied health professionals

**Professional Development**
- A full induction will be provided.
- Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities.
- Involved in one-to-one meetings with line manager monthly to discuss targets and outcomes achieved.
- Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
- Review yearly progress and develop clear plans to achieve results within priorities set by others.

**Knowledge, Skills and Experience Required**
- The ability to demonstrate understanding and empathy for the types of challenges faced by some CYP.
- A willingness to collect data to demonstrate and capture the impact of the role.
- Enthusiasm and drive to identify opportunities to shape and enhance CYP services in Hillingdon.
- Effective listening and communication style with the ability to build trusting relationships
