Band 3 Rehab Assistant - up to 25 hours per week
The West Early Supported Stroke Discharge (ESSD) Team are excited to advertise this permanent Rehabilitation Assistant post of 25 hours per week. The ESSD service plays a vital role in safe, supported & early discharge from the Hyperacute Stroke Unit at the Cumberland Infirmary and the stroke ward at West Cumberland Hospital and works with people in their own homes to increase their function following a stroke. We cover the ICC areas of Workington and Copeland and travel across these areas is required. We work collaboratively across the North Cumbria Stroke Pathway to ensure a seamless provision of stroke services, appropriately working alongside statutory and non-statutory partner agencies such as Adult Social Care and the Stroke Association. As a Rehabilitation Assistant in the team you will be pivotal in the implementation of evidence based specialist treatment programs for patients following a stroke, who may have a range of impairments and complex health needs. The therapy plans often include supporting patients to practice their normal daily activities such as personal care, meal preparation, support with medication and accessing their community in order to achieve their rehabilitation goals. You will work closely with patients and their families and carers in the implementation of their care plans. You will be working with a full multidisciplinary team comprising of occupational therapists, physiotherapists, speech and language therapists and neuropsychologists.
The ESSD service works with the Workington and Copeland Integrated Care Community teams to provide a seamless transition of care from the acute hospital setting into the community. This will require the ability to travel across these areas, as well as to work flexibly to meet service needs. You will provide specialist rehabilitation with patients following a stroke, in their own homes and other community and clinical settings, and under the direction and supervision of qualified therapists. The post may involve weekend working.
