All roles on the bank are for as and when workers and therefore in particular may suit those already working part-time, with other commitments, or who want to work on a short-term basis.

**Job Summary**:
To participate in the organisation and provision of Community Mental Health services to the population of Hertfordshire via different locations (placements will be worked via one site).

The post holder is accountable for the delivery of high quality Community Mental Health Nursing Services to an allocated caseload, within frameworks agreed by the Trust.

To provide clinical supervision and advice on nursing procedures to junior nurses and support staff.

**Responsibilities**:
1. To participate in referral procedures and meetings, undertaking assessment, with other professionals, of referrals to the service. Implementation of an agreed plan of care, highlighting any unmet needs.
2. To be accountable for the delivery of care to an allocated caseload, acting as Care Co-ordinator/Manager when required. This must include the active involvement of the service user in the care planning process.
3. To undertake monitoring and evaluation of care within the Care Programme Approach framework with the involvement of the service user and the multi-professional team, including GPs and external agencies.
4. To maintain comprehensive, up to date, and accurate clinical records, including Risk Assessment, in line with NMC requirements and Trust policies, and to maintain timely records of activity and performance data as required, utilising the Trust’s electronic patient record system.
5. To ensure full compliance with aftercare requirements such as Care Programme Approach, Section 117, CTO, Guardianship and Social Care requirements.
6. To participate in the CMHT duty system acting as the first point of contact for referrals and enquiries to the team.
7. Where necessary to administer prescribed medication or to supervise its administration and with the service user assess for desired and non-desired effects.
8. To take on additional responsibilities/lead roles on behalf of the team e.g. Clozaril clinic, education, health and safety etc.
9. To be fully conversant with the requirements of the MHA (2007), the Code of Practice and other relevant legislation.
10. To generate effective communication and liaison within the multi-professional teams, creating and maintaining appropriate and harmonious links with other professionals, team members and departments. Attend team meetings as required.
11. To provide and facilitate specialist help and advice to other professionals, agencies and carers.
12. To create and be involved in mental health promotional activities.
13. To provide effective professional supervision/mentorship of more junior staff and learner nurses as required.
14. Initiate/participate in training programmes, workshops, teaching sessions for Trust staff, learner nurses, local agencies and special interest groups as required.
15. To provide a flexible service, participating in an on-call system when required.
16. To comply with the NMC Code of Conduct and Trust policies and procedures at all times.
17. To ensure effective/economic use of resources available.
