**How you'll make a difference**:
As an Early Help Co-Ordinator you will make a difference by undertaking a range of tasks and providing key elements of early help preventative work to complement and support the existing full range of services, aiming to ensure that vulnerable disabled children and young people can achieve positive outcomes.

We have 2 posts available:
1 permanent, part time post for 18.5 per week. Reference SGC0055

1 12 months, limited term contract for 37 hours per week, Reference SGC2077

**What will you be doing?**
- You will review Care packages and oversee regular reviews of care plans and make sure that needs are being met, and ensure positive outcomes are being achieved.
- On a daily basis you will update information to ensure that any new needs are responded to at the appropriate level, and refer cases back to social workers within the team. You will include Children and Young people in their review and ensure that they have copies of their personalised plans in a format that is most suitable for them.
- Your role will include completing transition assessments, with the support of Social Workers, to identify pathways to adulthood and Mental Capacity assessments and provide EHCP contributions for young people with disabilities, helping families explore or identify support via their extended family or via the local offer. All activities undertaken will include visiting various educational settings, family homes or places of service provision.
- You will organise and chair review meetings, gather multi agency information, prepare and share reports in a timely and professional manner, as well as be able to represent young people, team and service at a variety of internal and external meetings and panels as relevant to the families allocated to you.

**What we need from you**:

- You will have 5 GCSEs or equivalent (inc English and maths) and considerable experience of working in a Statutory setting (Children and families or education), which should include an assessment function and experience of preparing written records and reports.
- We require you to have effective interpersonal, communication and relationship building skills, including handling difficult and sensitive issues.
- It is essential you are able to carry out good quality written reports, communicate findings to families and other relevant professionals and service providers.
- You must be able to demonstrate a working knowledge of regulations and anti-discriminatory practice towards those with protective characteristics, or be willing to learn and develop in these areas.
- You will hold a full clean drivers licence and access to a car to travel to a range of locations including home and school visits.7

Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post
