**The role of our Outreach Support Workers**:
Our **Outreach Support Service** provides care and assistance to individuals living in their own homes — people who are largely independent but need a little extra help to manage daily life.

You’ll play a vital role in empowering people to stay connected, safe, and confident in their own communities.
- Support individuals in their own homes with day-to-day living (meal prep, medication prompts, household tasks, budgeting).
- Promote independence and dignity while encouraging decision-making and self-care.
- Support people to access community activities, attend appointments, and maintain social connections.
- Provide emotional reassurance and companionship.
- Record and report progress in line with care and support plans.
- Work flexibly across Northumberland and Tyne & Wear to meet individual needs.

As well as providing physical and emotional support, you may be needed to provide personal care, assist with taking or administering of medication and support individuals with specific healthcare needs.

**Experience our Outreach Support Workers need**:

- Driving preferred.
- Able to support wheelchair user
- Innate ability to be compassionate and patient.
- Previous experience with supporting independent individuals in domiciliary care setting.
- A clear written and verbal communication technique, along with basic IT skills and numerical ability.

**Location Specific requirements**:

☒Able to support wheelchair user

☒Overnight and weekend shifts on a rota

Typical shifts are 9am starts working throughout the day until around 6pm. Supporting multiple people throughout the day providing no less than 3 hours per session or working 6 hours with one person.

You'll be required to work throughout our Outreach locations in Newcastle, Gateshead and North Tyneside. Support people with finances, cleaning, activities and events as well as health appointments
