An exciting opportunity has arisen for an enthusiastic, creative and experienced health practitioner to join the Homefirst Team in Carrick. This position is open to a Physiotherapist; Occupational Therapist or registered RGN who has experience of working in the community and wishes to develop his/her rehabilitation skills whilst being part of a dynamic, progressive, integrated multi-agency, multi-disciplinary team. The successful applicants will be based in Falmouth Health Centre.

The Homefirst Team is part of the integrated community team in Carrick which includes registered and non-registered nursing, therapy and administrative staff who work closely with partnership organisations to support the intermediate health needs of our local population.

You will be responsible for assessment, planning and implementation of care and assist in the interpretation of information relevant to the patient’s condition/progress, identify problems and needs of patient and set realistic goals.

You will need to have excellent organisational skills combined with a wide knowledge base and experience of working in older person’s services.

We will offer a supportive working environment in which to develop your clinical and interpersonal skills, a structured in-service training programme; appraisal and supervision system. CPD is actively supported through regular supervision and staff training.

To provide a high standard occupational, Physiotherapy and Nursing to the service, performing advanced assessments of patients including those with highly complex presentations, and from this formulating a diagnosis and treatment plan.

To operate as an advanced practitioner being first contact for urgent requests to help both prevent hospital admission and facilitate early discharge.

To utilise advanced skills in community assessment and management of the acutely sick patient.

To provide leadership for support staff and junior staff through supervision, training and appraisal.

To participate in the planning and development of occupational services within the designated team.

Model and deliver excellent evidence based clinical care, working as an autonomous Practitioner.
