**Key Responsibilities**: Clinical Assessment and Diagnosis: Conduct comprehensive assessments of patients in the SDEC unit, including physical examinations, ordering diagnostic tests, interpreting results, and making differential diagnoses. Care Management: Develop and implement treatment plans, including prescribing medications, non-pharmacological treatments, or interventions as required, in accordance with professional and trust guidelines. Advanced Decision-Making: Utilize advanced clinical reasoning to determine the need for patient admission, discharge, or referral to other services, ensuring safe and appropriate care pathways. Collaborative Working: Work closely with the multi-disciplinary team, including consultants, junior doctors, nurses, and allied health professionals, to ensure seamless, high-quality patient care.
**Patient-Centered Care**: Ensure all care is delivered in line with best practice, incorporating shared decision-making and patient involvement in their care plan. Leadership and Supervision: Provide leadership and mentorship to junior staff, promoting the development of advanced practice skills within the unit. Clinical Governance: Participate in clinical audit, quality improvement initiatives, and service development, contributing to the continuous improvement of the SDEC service. Education and Training: Actively participate in the education and training of staff, students, and junior doctors within the unit, ensuring the dissemination of up-to-date clinical knowledge and practice.
**Professional Development**: Maintain clinical competence and continued professional development through reflective practice, attending relevant courses, and engaging in supervision and appraisal. Qualifications and Experience: Essential: o Registered healthcare professional (e.g., Nurse, Paramedic, Allied Health Professional) with current NMC/HCPC registration. o MSc in Advanced Clinical Practice (or equivalent). o Independent prescribing qualification.
- Substantial post-registration experience in an acute/urgent care setting, including previous experience in emergency medicine, acute medicine, or similar areas. o Proven competence in clinical assessment, diagnosis, and management of acutely unwell patients. Desirable: o Experience working within a Medical Same Day Emergency Care (SDEC) Unit or similar service. o Experience in leading and managing service improvement projects.
- Experience in teaching, mentoring, or supervising junior staff. Skills and Attributes: Excellent clinical reasoning and decision-making skills. Ability to work autonomously and manage own caseload. Strong communication and interpersonal skills, with the ability to work collaboratively with a multi-disciplinary team.
