The post holder will be practising autonomously as an advanced practitioner within the Crisis Response Team providing patient-centered clinical care. This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice throughout the directorate.

The post holder will support new ways of working that focuses on efficient and patient focused service, and will ensure the safe treatment, referral and discharge of patients with undifferentiated and undiagnosed presentations in their area.

The Integrated Care Business Group within Stockport Foundation Trust is seeking an Advanced Clinical Practitioner (ACP)
- (Nursing /AHP/Therapist) to work within the Crisis Response Team.

We also aid early supported discharges with the innovative concept of a virtual ward to allow patients to receive the care and monitoring they need at home safely and conveniently.

The post holder will deliver an 80% clinical component to their role and 20% related to appraisal, clinical audit, teaching, self-development and research.

Undertake independent comprehensive assessment and management of patients to incorporate physiological and pharmacological considerations, making multifaceted complex clinical decisions regarding patient management and clinical outcomes. This assessment will be carried out within 2 hours of the referral to CRT.

The post holder will be responsible for providing advanced clinical care and expertise in care delivery including judgement, discretion and decision making to patients as per individual scope of practice and through the advanced clinical competency framework provided by the MSc programme.

The post holder will provide a seamless, high quality service from referral through to assessment, diagnosis, treatment and review, referring to other specialists as required. Provide guidance to staff, patients and their families and carers on pathway navigation.

If successful this vacancy will require an enhanced DBS check at a cost of £38. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
