Respiratory Practitioner

The post holder will work as a Respiratory Nurse/Physiotherapist assisting in the assessment, planning, delivery and evaluation of evidence-based care for patients with chronic lung disease within a hospital and community setting. The role will involve reviewing patients in the local AED or ward areas, facilitating an early/supported discharge where appropriate and safe to do so, and following them up at home. They will also be responsible for monitoring acutely unwell patients within the home environment to avoid unnecessary admissions, as well as undertaking oxygen assessments and supporting the Consultant in the multi-disciplinary clinics.
The service is operational 24 hour a day, 365 days a year, with an overnight on-call rota which the post holder will be required to participate in.
