Accountable to: Senior PCN Support Worker
Location: South Tyneside
WE are currently recruiting on behalf of Age Concern Tyneside South (ACTS), a registered charity which promotes the wellbeing of older people living in South Tyneside, improving their quality of life and helping them maintain their independence.
This service will enable older people with mental health issues to actively participate to making positive changes and reduce the risk of people not being able to access the services they need.
We have an exciting opportunity for a passionate and motivated Information Senior PCN Support Worker. This post will work in the community in partnership with the PCN team, and provide appropriate support, based on the needs of the individuals referred to the service. You will be responsible for achieving the agreed targets for the service, ensuring all activities are undertaken to the expected standards, including line management the PCN support workers.
You will provide consultations with clients to develop person-centred action plans, assist them to access mental health care and signpost to other services that will benefit them. You will also develop relationships with relevant contacts within the PCN and other stakeholders where appropriate.
You will have experience of working in Health and Social care or information and advice and working with people with mental health issues. You must have excellent communication skills, effective organisational skills and the ability to produce high quality reports.
