Ty Anwen is a residential mental health service in Caerphilly. We provide person centred support for up to 16 individuals with serious and enduring mental illness and complex needs which may include support with self-harm. The team at Ty Anwen work to arecovery model of care and offer support that provides effective therapeutic boundaries. They provide clearly identified outcomes, thereby working towards recovery and facilitating independence.

The role of the Mental Health Project Worker is to empower and support individuals we support, working flexibly to improve performance and outcomes for people who use our service. You will work as part of a team to develop, implement and review high-qualityperson-centred care and support plans based on individual needs, preferences and aspirations when required.

**How can I make a difference?**

As a Mental Health project worker you will be involved in day-to-day support including;
- Supporting individuals to live independently within a care setting.
- Support, prompting and administering medication
- Engagement in social activities
- Completing and updating support plans, risk assessments and risk management plans.
- Carrying out supervisions
- Completing assessments from referrals
- Be responsible for the service in the absence of the manager
- Undertake relevant training for the role
- Liaise with Social Services, Benefits Agencies, Housing department and other relevant agencies

**Who are we looking for?**

Our Team will possess:

- A genuine desire to help people and make a difference
- 3 years’ experience in a similar role
- Have or be willing towards completing a QCF in Health and Social Care
- An understanding of housing options and the process of re-housing to support individuals when they move on from Ty Anwen
- An understanding of the challenges that many of our service users face
- Keen listening and communication skills
- Empathy, compassion and a caring nature
- Flexibility including working evenings/night bank holidays and weekends
- Respect, integrity and dignity for our client groups
- A non-judgmental approach to providing care and support
- Ability to complete and regularly review relevant paperwork such as support plans and risk assessments

- This is a flexible relief opportunity
- The hourly rate for this role is £12.46 making the annual salary £23,974 (FTE)
- 12.07% relief holiday payment top up
- Shifts include day and night shifts.
