We are looking to recruit an experienced General Practice administrator to take on a Care Coordinator role to support the prioritisation of patient care. This is a full time role at 37.5hours per week but we can be slightly flexible on that as well as how the hours are worked across Monday to Friday.

This is a diverse and challenging role that is integral to the quality of service and care we offer our patients. The work will include reviewing incoming calls and online medical enquiries to identify urgency of care required. You’ll be liaising with our clinicians to ensure safe and efficient decision making. You’ll then be updating, advising and signposting patients to the most appropriate care for them and coordinating that care. You’ll also need to regularly review appointments booked to ensure efficient and consistent care most appropriate for the patient and clinicians, and ensure personalised care for the patient. You’ll also identify patients who may benefit from a personalised care plan and work with the Wellbeing Team to create this and implement it. This role is derived from the Primary Care Network and will need to work with the criteria set for PCN Care Coordinators, including personalised care and tackling health inequalities.

You will need to be able to regularly work within our Patient Services Team to help ensure sufficient cover for all key administrative support work for the Practice.

This is a senior administrative role so you should be highly experienced in a range of administrative skills and with previous experience in a GP surgery have knowledge of complex appointment booking and have developed excellent communication skills with patients/service users. You should also be confident working within a multi-disciplinary team. Any previous experience or knowledge of personalised care and shared decision making would be beneficial but not essential as training will be given.

**Prerequisites**:

- Experience working as an administrator in a GP Practice
- Experience working with appointment booking in a GP Practice
- Experience of working closely with patients and their carer(s)/parent(s)/guardian(s)
- Knowledge of Confidentiality for patients and the workplace
- Knowledge of Safeguarding
- Knowledge of Medical Terminology and clinical systems and processes

Cricketfield Surgery is a friendly and caring primary healthcare practice that believes in nurturing the wellbeing of its staff as well as patients. An established and thriving practice, operating for over 25 years, with a current list size of around 10250 that is steadily increasing. Newton Abbot is a bustling market town surrounded by beautiful countryside with the coast and Dartmoor nearby, as are the vibrant cities of Exeter and Plymouth so it’s a fabulous part of the world to be based. We also work closely with other GP practices in the local area within our Primary Care Network and wider Federation. We support the training of both postgraduate GP trainees, medical students and nurses and we are often involved in research projects. We currently have 3 GP Partners, 1 Nurse Partner and 5 salaried GPs, and then have 30 other staff members across a variety of clinical and administrative roles. We are a busy practice who work hard but also have a sense of fun and aim to make all team members feel integral to its success.
