The Integrated Discharge Coordinator
- is expected to support the integrated system to facilitate safe and timely discharge.

The post holder will have the responsibility to work as an integrated member of a system-wide Health and Social care multidisciplinary team to ensure the hub is meeting the patient and service needs, providing expert advice to ensure effective delivery of the service in accordance with the service specification.

The post holder will work with the Integrated Discharge Lead to ensure effective integration and support teams to deliver high quality and effective care and ensure optimal flow through acute, interim and final discharge destinations.

To ensure Senior Managers are informed to of the appropriate trigger points for service escalation and ensure that all localities have agreed escalation level specific actions.

To maintain oversight of national and local policies, relating to Medically Ready for Discharge Transfers and reflected in Trust policies and procedures.

To maintain an overview of Surge and Escalation policies and procedures arising from Local A&E Delivery Boards.

To support with clinical operations and informatics, specifically with respect to data flow.

To participate on routine escalation calls, ensuring that agreed actions are implemented.

To identify areas within scope, suitable for audit to ensure that policy and process is being adhered to in practice and identify areas for improvement.

Case manage the discharge process for patients

Medical services have been delivered on the Whittington Hospital site for over 500 years. On 1st April 2011, Whittington Health was launched as a new Integrated Care Organisation (ICO) comprising The Whittington Hospital NHS Trust, NHS Islington and NHS Haringey. The Whittington and the community health services in Islington and Haringey are award winning organisations, delivering acute and community based health services to a population of 443,000 people.

The organisation is the biggest employer in the area, with over 4,500 staff; delivering healthcare not only on the acute hospital site which has 420 beds, but also from a variety of other community based health centres and through visits to patients homes across the boroughs to address the health needs of the local population. Whittington Health also works with other health, social care and voluntary sector partners to support patients and service users from their initial appointment whether it be with a community health team or at the hospital, all the way through to treatment and tailored after care.

Participate and contribute to the delivery of clinical quality and standards for practice.

Work collaboratively with stakeholders across the health economy to address any shortfalls in the current systems and ensure the highest standards of timeliness and overall patient satisfaction.

Draft reports summarising status on issues, appraising outcomes and providing progress reports.

Collate and present qualitative and quantitative information and lead appropriate analysis to make robust recommendations.

Analyse, interpret and present data.

Work across the system at times to review standards of practice, process and provide feedback to support staff.

Model the behaviours which promote the Trust’s values, including the promotion of a culture of equality.
