Patient Liaison Assistant

Patient Liaison Assistant required for rural General Practice. Responsible for receiving, assisting and directing patients and ensuring they access the appropriate service or healthcare professional in a courteous, efficient and timely way.
The post holder will provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.
25.5 hours per week over 4 days, working within a small, friendly and supportive team with extra hours as required to cover for holiday and sick leave.
MAIN DUTIES AND RESPONSIBILITIES
- Greet and note arrival of patients on computer system for appointments in a courteous manner
- Answering general enquiries, explaining surgery procedures
- making new and follow-up appointments with patients in person or by telephone
- Contacting patients at the request of doctors to make appointments and pass on messages
- Providing information about how to access hospital and other healthcare services to patients when requested
- Liaising with Community and Secondary Healthcare providers regarding the continuing care of patients.
- Ensuring that information about any unresolved or urgent matters are passed to colleagues at the end of shift.
- Scan ALL clinical post into the DOCMAN system and in patient records - ensuring post items are work flowed for the doctors attention
- Allocate non clinical post to appropriate pigeon holes and open all journals and place in education room (removing the previous issues)
- Scan policy and procedure documents into DOCMAN and circulate as needed
- Review all work flowed documents and undertake all actions as requested by doctors e.g. adding further coding or contacting patients
- Download Pathology results (Pathlinks) each morning and afternoon and deal with any unmatched results or those where the patient is not registered with the practice
- Review and complete all actions from Pathlinks once the doctors have seen the results
- Use the UHCW results reporting system (CRRS) to obtain further results and letters regarding patients as necessary and pass these to the doctors/nurses for information and action
- Inform doctors of any information received from patients or other healthcare professionals as a result of a request from a doctor
- Respond to and complete all requests on “Reception Tasks” and provide feedback to the requestor when required
- Deal with patients, colleagues and visitors to the practice in a courteous and efficient manner at all times
- Be responsible for own time management when completing other allocated duties that are needed for the safe and efficient running of the practice
- To work overtime on such occasions that the workload of the practice requires and to work agreed overtime to cover for periods of leave
- Assist in ensuring all areas of the practice (including consulting rooms) are neat and tidy and adequately stocked with stationary and equipment to enable to smooth running of the practice
- Suggest and support continuing service improvements and developments
Securing the premises on exiting at lunch limes and at the end of the day. Ensuring all windows and doors are locked and the alarm is activated
**Job Types**: Part-time, Permanent
Part-time hours: 25.5 per week
