The Integrated Care Homes Service (ICHS) is looking for a Band 7 Clinical Lead for the Dementia Outreach (DOT) element of the service. They will provide a high level of clinical support to the members within the DOT. The DOT element of ICHS comprises of RMNs, dementia specialist Physiotherapists and Occupational Therapists and support staff. This role can either be a AHP or a RMN who has experience of working with patients with complex dementia needs.

The clinical lead will be responsible for providing clinical leadership to the members of DOT, and to work alongside the other clinical leads within the service in providing specialist clinical leadership across all elements of ICHS. To increase knowledge and skills across all disciplines within ICHS to promote an integrated model of working ensuring that the patient receives intervention from clinicians with the appropriate level of knowledge and skills.

They will provide advice and support to staff around mental health requirements of clients within their remit, signposting to specialist mental health services as required.

The clinical lead is required to have a keen interest in making a difference to the specialist dementia provision to the residents in care homes and empowering care home staff to work alongside the residents in the ongoing delivery of interventions. Along with understanding the benefits that an integrated service can make to the residents and to the staff within ICHS.
