**Care Home Assistant Practitioner**
**MAIN PURPOSE OF THE ROLE**
To participate in the daily functioning of the care team in providing a person centred, caring and professional service to all Service Users in a safe and homely environment. Working under the direction of the nominated Nurse in charge, to provide medical care to support clinical decisions and to undertake the safe administration of medication. The nominated Nurse will either be on the same unit as the post holder, or on an adjacent unit. To work with and alongside seniors and care assistants to provide care and support with the physical and mental health needs of the Service Users.
**SUMMARY OF KEY RESPONSIBILITIES**
The responsibilities linked to this position can be summarised as follows:
- Ensuring the highest standards of person centred, caring, safe, responsive and effective care is delivered to all Service Users at all times.
- Working as a team player in support of the nominated nurse in charge and alongside care team who they will both support.
- Working under the direction of the nominated nurse-in-charge to provide medical care support in areas such as routine observations, body mapping, weight management etc.
- Carry out base line observations including, temperature, COVID testing, blood pressure, pulse, O2 saturate level as requested by the Nurse.
- Carry out venepuncture as requested by the Nurse.
- To undertake the safe administration of medication.
- Assist in maintaining hydration/nutritional needs of Service Users, monitor skin integrity and ensure prescribed positional changes take place.
- Ensure documentation is completed and recorded accurately in a timely manner
- Carry out and organise Service User reviews as and when required - support Nurses in care review meetings - CHC assessments.
- Maintaining and respecting the confidentiality of Service User’s, their relatives and all staff within the home as well as visiting professionals.
- Adhering to and promoting Safeguarding, Health and Safety and other protective measures at all times; acting on and reporting any concerns identified or raised in a prompt, professional and appropriate manner.
- Maintain professional knowledge and competence - develop areas of good practice for discussion with Manager/Deputy
- Assist in the development and implementation of each Service Users personalised plan of care.
- Assist with addressing shortfalls identified through quality monitoring audits.
- Complete internal assurance checks to ensure compliance and quality.
**KEY ACCOUNTABILITIES**:
To actively participate in, as well as to promote, the provision of high quality care to all Service Users as directed by the Lisbeth Care Home philosophy and within the parameters of policy and procedure, enhancing the collective safety, comfort and well-being of all Service Users.
To deliver and promote person centred care to the highest possible standards to meet individual and collective needs.
