An exciting opportunity has arisen for a Diabetes Education Assistant to join the Intermediate Diabetes Specialist Service at Whittington Health NHS Trust. The post is based with the community Intermediate Diabetes Specialist Team (IDSS).
This is a vibrant department which strives to keep up to date with research and technology.
You will assist with the delivery of virtual/face to face educational sessions and support the Diabetes team in planning and managing effective caseload. This will involve arranging patient appointments by telephone and maintaining
appropriate records. You will therefore need to work on your own initiative.
You will need to demonstrate excellent communication and interpersonal skills, the ability to work flexibly and reliably as part of the team. Excellent team working, organisation and time management skills and administrative skills are essentials.

The post is based across Haringey and Islington.

To manage and prioritize Diabetes education caseload as delegated by Registered Dietitian.

To coordinate, help set up and deliver structured group education sessions to patients with diabetes and their carers, this will be primarily “DESMOND” a group education for patients with type 2 Diabetes. If not, already a trained DESMOND lay educator, there will be an opportunity to complete this training.

To organise and facilitate group consultations

To encourage patients and their carers in an active approach to regain and maintain personal independence. This requires empathy, sensitivity, and good interpersonal skills.

An exciting opportunity has arisen for a Band 4 Diabetes service Education Assistant to join our community Intermediate Diabetes Specialist Service (IDSS) at Whittington Health. Within this role the post holder will deliver diabetes education and support in elements of diabetes care to patients in the community, as delegated by the Diabetes Specialist Dietitians and Diabetes Specialist Nurses.

The post holder will be required to work across Whittington Health community sites including health centres in both Haringey and Islington.

The post holder will work under the supervision of Registered Dietitians who lead patient education within the service. They will also support in coordinating other group consultations with other Diabetes Specialist Practitioners; this may involve being a facilitator alongside the clinician.

To support clinicians in reinforcing patients and their carers on their care plans as required, ensuring adherence with dietary treatment.

To actively participate in health promotion through promoting healthy eating, in line with government policies and local priorities, liaising closely with the public health departments.

To participate in continuing professional development, undertaking on-the job training and other training as required in the annual appraisal system and required within the role.

To comply with professional guidelines, Whittington Health NHS Foundation Trust, and departmental policies.

To be able to measure and weigh patients using anthropometric equipment.

To assess risk and manage this effectively within the clinical and community settings.

To participate in regular supervision and reflective practice.

To manage and prioritise patients in a variety of settings under clinician caseloads, as delegated by the Dietitian and to regularly feedback regarding patient progress.
