**Empowering customers to live independent, rewarding and happy lives in small units in the beautiful Exeter area of South Devon. rewarding and happy lives. That’s when it hits home.**

Due to the awarding of new contracts and company expansion, we have a number of fantastic opportunities for energised and creative Supported Living Co-ordinators like you to join our brilliant and supportive team.

Our mental health recovery service provides support to our customers who have maybe spent prolonged periods in hospital or institutional settings and are taking the next step back to totally independent living.

We provide support to customers in shared houses of up to 5 people, living as a community and sharing the chores with their fellow housemates.

You’ll help our customers identify their aspirations, and work with them to create plans that help them achieve these, in collaboration with clinicians and wider NHS services. You’ll work with colleagues to deliver on the plans you’ve created to achieve excellence, as well as learning new skills and tools to enhance your professional development.

Our aim is to support our customers to live their day to day lives as independently as possible, which means that no two days are ever the same because everything we do is “person centred” around our customers.

You might support with breakfast and their medication, attending college or the gym, taking part in community and social activities, as well as trips out, walking and other meaningful activities to give choices based on their needs.

Like our team, we’re sure you’ll find the role truly rewarding.

**Customer quote**
- “I am opening up more because I am starting to trust and believe that staff are really there for me.”_

**Be yourself at Home**

Home Group is committed to tackling prejudice and discrimination, not just for our customers, but for our colleagues too We’re super proud of our internal Diversity Networks and allies (Multicultural, LGBTQIA+ and Disability) which support colleagues from different backgrounds to be their true selves. We recognise and celebrate our differences, together we make Home Group a great place to work

**About you**
You’re passionate and motivated. To you providing care and support is more than just a job, you make every customer feel valued and respected. You’re compassionate and use your unique empathy and creativity to help our customers reach their full potential.

Treating our customers with integrity, you ensure that they always maintain their dignity. You’ll help change our customer’s lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. You’ll create bespoke and personalised support plans with and for your customers. Each plan will look to meet our customer’s individual potential and long-term goals, making sure they meet safeguarding and confidentiality requirements.

You’ll also have responsibility for coordinating apprentices and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.

You must be able to use IT for recording and evidencing actions. To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe, as we will be happy to offer additional support when necessary).

To make sure our customers are well supported, it’s important that you have worked in a customer focused environment before. You have a great understanding of professional boundaries, responsibility for safeguarding, risk assessment, confidentiality and have experience in case management.

Don’t worry, we won’t drop you in the deep end, we’ll help to you understand how things work at Home Group, through a robust and full induction and training package

It would be amazing if you have worked in a mental health recovery environment before, however it’s your “can do” positive attitude and enthusiasm that are critical to make this role a success. We can nurture your talent, just let us know in your covering letter why you have a passion for working here.

As this is a community-based role, though your day to day base is in Torquay, you need to be willing to travel across Devon and the South West region. If we need you to travel by car, you must have business insurance cover. Don’t worry, we’ll pay you expenses at HMRC rates to cover this use.
- When you join, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don’t have a transfe
