Assistant Practitioner

The Assistant Practitioner will ensure the delivery of effective, efficient and clinically safe community nursing services across Derbyshire. It is expected that applicants will either possess or be working towards the Assistant Practitioner qualification. The post holder will be providing care and support to patients in their own homes for the out of hours community nursing service based in Chesterfield. Following appropriate training, the AP will work without direct supervision, undertaking delegated tasks and reporting back to the relevant member of the clinical team. The post holder will perform and assist in the provision of holistic, high quality, evidenced based health care to a defined local population, working within an appropriate supervisory and monitoring framework, as appropriate to clients across a wide spectrum of clinical conditions within the service provided by DHU Healthcare CIC.
**Areas of responsibility will include but not be limited to**:
- Be responsible for the implementation of treatment and the development of programmes of care for designated patients and provide feedback / escalate discussion regarding meeting patient need through supervision/monitoring processes.
- To provide nursing care to individuals making use of current evidence to inform the delivery and promote the development of clinically effective care.
- To ensure a continuity of care in accordance with the care programme, seek support from other specialist practitioners/operational managers as required.
- Identify need, and provide advice, support and demonstration to the patient/carer on the use of the equipment to ensure a personal duty of care and compliance with health and safety policies.
- To monitor and report to senior nurse effectiveness of equipment and aids in patients’ homes to maximise the use of resources to enable care packages to be delivered.
**You will need to meet the followingessentialcriteria**:
- NVQ Level 2 or equivalent
- 4 GCSE’s or equivalent, including Maths and English
- Completion of Care Certificate
- Previous community healthcare experience essential minimum of 5 years in similar role
- Experience of wound management, catheterisation female/male/suprapubic and knowledge of sepsis and sepsis score.
- Hold the Assistant Practitioner qualification or be willing to work towards the qualification.
