The Keyworker Function will support children and young people with a learning disability, autism or both, with the most complex needs to access the right support at the right time.

A fantastic opportunity has arisen for a proficient and highly motivated Individual with a background in working with children and young people and their family/carers to lead a new team working with children and young people (up to the age of 25) with a learning disability, autism diagnosis, or both, who are already in hospital, or who are at risk of a hospital admission due to their mental health. You will lead a team providing help and confidence to those you are supporting to access the right help across our complex systems in order to empower people and help them to achieve their potential in life.

**This post holder will**:
Management duties for a team of Keyworkers working across BaNES, Swindon and Wiltshire.

Manage a caseload of children and young people (CYP) who have been identified as requiring a keyworker. The caseload will allow sufficient time for the personalised support required within cases.

Facilitate the provision of emotional and behavioural support and informed advice to CYP and their families/carers based on knowledge and experience of working with CYP, families and carers with complex needs.

Create and encourage a culture which prioritises keeping CYP within the home and their local community by communicating with a range of teams, services, and staff from a range of statutory and non-statutory organisations.

Build and maintain positive, trusting, and appropriate relationships with CYP and their families/carers.

Work in partnership with organisations within the integrated care system (ICS) and in particular with Education, Social Care and Healthcare.

Develop and maintain a strong, positive relationship with children and young people with a learning disability, autism or both, who are inpatients or at risk of being admitted to hospital, to ensure children, young people and their families get the right support at the right time with a personalised plan and that local systems are responsive to meeting their needs in a holistic and joined up way.

Support the Lead Keyworker and Head of Keyworking Function in the delivery of the service across the ICS.

BSW ICB have adopted agile working so this means the successful applicant will predominately be working from home but may be requested to work in any one of the ICB’s office bases across BaNES, Swindon and Wiltshire. You will also have a contractual base within the BSW ICB office where your team is based.

**This post holder will**:
Split time between Keyworker Team Lead (50%) and Keyworker (50%)

**As a Keyworker Team Lead responsibilities will include**:
To manager a team of Keyworkers and administrative support to deliver the Keyworker Function.
To provide operational management support to the Keyworker Manager.
To report to the Keyworker Manager and Keyworker Specialist on progress of the implementation of the Keyworker Function, with compliance with pilot metrics and collate data as appropriate to support evaluation.
Report to the Keyworker Manager on the demand and capacity on the Keyworker Function.
To escalate risks and opportunities via the agreed operational structure for the Keyworker Function.
Manage a caseload including leading or supporting on highly complex cases.
Advise the Keyworker Manager on allocation and prioritisation of cases within the team taking into account the differing skillset and experience within the team.
To monitor referrals, caseloads, and outcomes of the Keyworker Function.
To escalate risks and issues to the Specialist Keyworker and Keyworker Manager.
Contribute to any future required reporting or documentation relating to the Keyworker Function.
