Full Time Young Persons Support Worker - Leaving Care - Youth SupportFull Time Project Worker - Children and Young People - SWANSEA (Step Down Provision) Your New CompanyYou will be working for a well-established children's support company who focus onnurturing children and young people with emotional behaviour disorder back into the community, from within the care sector. This step-down provision is a transitional home for young people to allow for more independence and individuality. It builds confidenceand uses carefully thought out care plans to help through this period out of care. They may have backgrounds from addictions, neuro-diversities, SE and DV. It can be an active home but your main role is to be a role model, and guide the young people throughtheir care plans - oh, and cook a fresh meal with them every Sunday If you want more information on this role please get in touch by applying below Your new roleTo provide outstanding care to young people while applying their individual care plansEngagein fulfilling activitiesLiaising with families and support workers to ensure the best possible independent care plans are provided and carried outProactive approach to education and activity to improve key skills and self confidence.Ensure clear communicationwith individuals via a range of methods and inspire them to carry out any tasks.Engage in individual's transitioning periods by nurturing them to become a functioning member of wider societyTo make and maintain accurate records of activities and tasks carriedout, including any incidents that may occurCarry out general housekeeping dutiesApply safeguarding and protection to vulnerable individuals and ensure the safety and wellbeing of everyone you supportComply with GDPR and Health and Safety regulationsShiftsare 24 hours and lone working so experience is essential.
