Young Persons Leaving Care Support WorkerYour New Company You will be working for a well-established children's support company who focus on nurturing children and young people with emotional behaviour disorder back into the community, from within thecare sector. This step-down provision is a transitional home for young people to allow for more independence and individuality. It builds confidence and uses carefully thought out care plans to help through this period out of care. They may have backgroundsfrom addictions, neuro-diversities, SE and DV. It can be an active home but your main role is to be a role model, and guide the young people through their care plans - oh, and cook a fresh meal with them every Sunday If you want more information on this roleplease get in touch by applying below Your new role To provide outstanding care to young people while applying their individual care plans Engage in fulfilling activities Liaising with families and support workers to ensure the best possible independent careplans are provided and carried out Proactive approach to education and activity to improve key skills and self confidence. Ensure clear communication with individuals via a range of methods and inspire them to carry out any tasks. Engage in individual's transitioningperiods by nurturing them to become a functioning member of wider society To make and maintain accurate records of activities and tasks carried out, including any incidents that may occur Carry out general housekeeping duties Apply safeguarding and protectionto vulnerable individuals and ensure the safety and wellbeing of everyone you support Comply with GDPR and Health and Safety regulations Shifts are 24 hours and lone working so experience is essential.
