Community Mental Health Practitioner

We are looking for an experienced mental health professional (nurse, social worker or occupational therapist) to join the Recovery and Wellbeing Team based in Castle Point. We are a dynamic, multi-disciplinary service, providing a range of specialist interventions using an integrated person-centred approach to adults who are residents of the Castle Point area. We work with those who have a diagnosis of a mental health condition and often have complex presentations. These can include illicit substance/alcohol use or physical health concerns and sometimes be further compounded by complicated familial and/or social dynamics and factors.
You will be expected to, assess, plan, implement and evaluate all aspects of care. You will be part of a vibrant and experienced multi-disciplinary team and possess an understanding of how multidisciplinary teams need to function. You will also be expected to liaise with other statutory and non-statutory agencies in the provision of care and have the ability and confidence to engage service users, carers and relatives with empathy and compassion whilst working towards an individual's recovery goals.
The team's operational working hours are Monday to Friday 9am to 5pm and the team base is in Canvey, although at times other trust bases/alternatives can be used depending on the situation and team priorities.
