This is a new and innovative service led by the local NHS, councils, Public Health, GPs, voluntary and community partners and people with lived experience of mental illness. Ensure that at the start of a person's recovery, wherever and however a person accesses care they are met with compassion, understanding, and supported with both their physical and mental wellbeing. Build teams around communities that are close to people's homes, by working with our partners including the voluntary sector, making sure people's recovery is close to their personal support networks. Provide people who are working across these physical and mental health services with the right support and training, making sure we have creative career options across the area. **Main duties, tasks & skills required**: The role will work closely with secondary mental health and primary care and ensure good communication between services. This will support to staff within the Primary Care Network and ensure engagement within the local community mental health team and work with Primary Care stakeholders in South East area to support and help deliver the transformation work around "No wrong door". By working closely with the secondary care these roles will provide a link between primary care services and secondary care for the Mental Health Team and review the processes of referrals between Primary Care and secondary mental health care; helping to ensure robust systems are in place. The roles will provide support patients in PCNs with guidance on how therapeutic interventions to service users in a primary care setting can be navigated to others or dealt within Primary Care; planning and coordination of service users' care in primary care, including those on the SMI list liaising with teams and agencies as required; and to provide signposting to appropriate agencies for ongoing support. **About us**: Hampshire and Isle of Wight Healthcare NHS Foundation Trust is a newly established organisation, bringing together expertise from across mental health, learning disabilities, community, and physical health services.
