An exciting opportunity has arisen for a motivated and committed healthcare assistant to support the clinical nurse specialists in the colorectal/urology stoma care team.

The post holder will join a flexible, enthusiastic team in a forward thinking department who are committed to working as ‘one team’ for the benefit of patients working across the three health board sites, including Morriston Hospital, Singleton Hospital and Neath Port Talbot Hospital as well as Community visits.

We are looking for someone with excellent communication and interpersonal skills, to support a team of five clinical nurse specialists caring for patients with colorectal/bladder cancer, inflammatory bowel disease and patients with stomas, assisting with both clinical and administrational tasks.

The main duties of the post are to support the colorectal/urology stoma care CNS team with the care of patients undergoing treatment for colorectal/urology cancers and stoma forming surgery and provide administrative support for the colorectal/urology stoma care CNS team, including facilitating outpatients appointments, managing patient calls, ensuring continuity of care with other healthcare providers.

The HCSW will work under the direction of the Colorectal & Urology CNS team in order to provide the highest possible standards of stoma care and support for patients within the hospital and community setting

The post holder will act on their own initiative when delivering delegated stoma care, but supervision from the qualified nurse will be available.

To maintain patient confidentiality at all times

To work alongside the Colorectal & Urology CNS and other Health Professionals to provide stoma care.

To communicate with patients, relatives and health care professionals.

To communicate via telephone & respond to queries.

Ensure a high standard documentation is maintained.

To liaise with clerical staff.

To undertake a range of clinical care as delegated by the CNS.

To teach patients basic stoma care autonomously.

To assess patient conditions and to report any problems to the CNS.

To develop a supportive relationship with patients.

To review established stoma patients in other establishments and their own homes, to assess if there are educational needs.

To maintain the supply of post operative stoma stock in department and ward areas.

To provide stock and literature for in-patients.

To communicate effectively with patients, relatives and staff.

To maintain confidentiality at all times.
