- Positions available at are full time and part time - minimum contract is 22.5hrs. Please note, you must have a UK driving license for this post. _**_We understand petrol / diesel costs are significantly higher at the moment, we do support with mileage costs for this post. _**
We have exciting opportunities for Assistant Practitioners who have a passion for providing rapid intervention in a patient's own home and supporting recovery from acute episodes of illness.
Are you a dynamic individual who is able to work with everchanging caseloads to assist in avoiding hospital admissions wherever possible? Supporting new development such as Urgent community response including falls, End of life and preventing deconditioning
- UCR work requires a 2 hours turn around to enable the person to remain at home. Currently other developments such as hospital at home will be coming online to further support the patients at home.
The Rapid Response team ethos is to provide a holistic approach to ensure that the patients care, and rehab needs are met within their own environment.
