We have an exciting opportunity to recruit three full-time Mental Health Practitioners to work within the Rotherham area. These roles will work within one of six Rotherham Primary Care Networks providing triage, assessment and mental health advice in a timely way working as part of the surgery MDT.
The introduction of these roles is part of a wider transformation and expansion of community-based mental health services set out in the NHS Long Term Plan. The practitioners can access and draw down on secondary mental health services and help provide a “bridge” between primary care and secondary mental health providers. Each practitioner will be embedded as part of the Primary Care Network Multi-Disciplinary Team working alongside other roles, they will also be part of the community mental health Multi-Disciplinary Team (MDT) with ready access to a wide range of support and expertise across both MDTs. It is envisaged that the focus of a practitioner’s work will generally be people whose needs cannot be met by IAPT services but who may not need ongoing care from secondary care community mental health services. The practitioner can facilitate onward access to other secondary care-based services, or third sector services integrated within the mental health pathway.
To provide a lead role in supporting the PCN in triaging mental health concerns raised by on the day patient requests, providing highly specialist advice and support within GP surgeries.
To review patients presenting with mental health issues, consider a range of options regarding treatment interventions providing them with highly specialised advice concerning care when appropriate and liaising with GPs and MH teams.
To offer brief interventions as needed e.g. problem solving, decision making, building resilience, building on personal strengths, risk de-escalation, medication advice and prescribing, if a registered NMP.
Working as a member of the primary Care MDT and taking responsibility for the development and implementation of specialist mental health training to the PCN.
To provide a navigation role to help GP’s make appropriate referrals to the relevant community and secondary care services - bridging the gap between primary care services such as IAPT and secondary care, working in partnership across the system.
To be confident working as the sole mental health practitioner with remote support from a pathway and supervision lead within the Trust.
To act as the first point of contact for GP’s, to offer mental health advice, support and signposting to appropriate agencies.
