We are looking for an enthusiastic, motivated individual to join our great team of Community Advanced Practitioners. They will be well organised, flexible, reliable and motivated. They will also need to be a team player and collaborate with colleagues from different professional backgrounds both in statutory and voluntary sector organisations.

This is an exciting time to join the team and get involved in shaping the future community focused care pathways. The demonstration of expert clinical leadership is highly regarded and essential in progressing the home first agenda. If you feel you have the skills and experience and relevant scope of practice to further enhance, strengthen and advanced practice within the community setting we would like to hear from you. The post holder will work in partnership with the Team Leader, Service Managers, Therapists and other staff within intermediate care and or community teams.

The post will also work in partnership with the Consultant in Elderly Medicine, GP, Community Matron, Case Managers, social care staff and the wider multidisciplinary team to provide personalised care across intermediate tier services and services in the community including the voluntary sector.

We are always looking for enterprising and innovative approaches to the way we provide our services. We are a national centre for telemedicine and introduced telehealth to the UK offender healthcare sector and also provide the service to patients in their care homes. Community matters to us and we are supported by 400 dedicated volunteers and have strong links with Bradford University, Craven College and Leeds City College to ensure we inspire the workforce of the future.

We want to attract staff who embrace our Right Care behaviours of compassion, a commitment to quality of care and working together for patients we want to make these part of our DNA.
