**As a Senior Care Coordinator, you will play a vital role in ensuring the delivery of high-quality care services to our clients. You will be responsible for coordinating and overseeing the care provided, assessing client needs, developing personalized care plans, scheduling appointments, monitoring client progress, and maintaining accurate records.**

**Proven experience in care coordination or a similar role. Strong organizational, multitasking, communication, and interpersonal skills are essential. Proficiency in using office software is required, along with knowledge of healthcare regulations and best practices.**

**While this role is predominantly office based (Manchester), you may be required to travel to meet clients and also the care givers at their location of work.**

**Duties**:

- Coordinate and oversee and ensure that extremely high quality standards of care are delivered and maintained by working closely with the Registered Manager.
- Assess client needs and develop personalized care plans
- Responsible for office administration tasks including being the first point of contact for all business enquiries, dealing with them effectively and efficiently.
- Responsible for managing Care Workers including helping with recruitment and training
- Schedule efficiently and effectively the delivery of exceptional quality care to clients per their care plans through planning and managing of rotas for care workers-Schedule and coordinate client appointments and visits.
- Monitor and evaluate client progress and adjust care plans as needed
- Effectively Communicate with clients, families, and healthcare professionals to ensure effective care coordination
- Responsibility in ensuring effective and efficient record keeping that guarantees; accurate invoicing (by Finance department), handling of any complaints and also necessary in adding for CQC audits/inspections.
- Weekly reporting on KPIs for sales, care workers and compliance

**Skills and Qualifications**

- Currently a Team Leader or Care Coordinator with Minimum 2 years experience within Domiciliary Care and possess an excellent understanding CQC care Standards built upon the five Key Lines of Enquiry (KLOEs) in that our service is safe, effective, caring, responsive and well-led
- Skills to onboard, retain and maintain clients.
- Strong organizational and multitasking skills
- Excellent communication and interpersonal skills
- Proficiency in using PC / IT skills (Microsoft Office Suite: Word, Powerpoint, Excel)
- Committed, flexible and hard-working
- Ability to work independently and as part of a team
- Hold a valid full UK driving licence

**Salary**: £25,000.00-£25,500.00 per year
