Everyone in Dorset deserves to live well. That’s why our NHS organisations, councils, public services and voluntary and community partners are working together as an integrated health system (ICS). We are joining up to tackle things that affect our health and wellbeing, make a real change, and improve things for our communities.

The successful postholder will have a key role within the Ageing Well Programme, acting as a conduit between BCP Adult Social Care and other ICS partners, to work on a range of complex strategic issues that support the development of local arrangements across organisational boundaries, and which will result in better outcomes for residents across the BCP area.

Priorities within the Ageing Well Programme include developing models of anticipatory care, plus the delivery of virtual wards and hospital at home services, the development of two-hour community response services and a two-day reablement response.

This fixed-term (12 month) post is fully funded through the ICS and represents an ideal opportunity for practitioners from either a social work or allied health professional background to help improve the experience of local people by delivering an integrated health and social care response to address their needs.
