**What's the role all about ?**:
This role will support our flourishing partnership with Derbyshire - Chesterfield and Royal Derby Hospital sites**_._**

NHS England recently identified that Virtual Wards are a high impact intervention that can support the NHS this winter.

**The role**

You will lend your clinical expertise to facilitate the referral process so that more patients can get home to their loved ones more quickly.

Tasks will include:

- Undertake initial referral assessments with patients.
- Support referring clinicians to finalise treatment plans quickly and effectively.
- Identify opportunities to diversify the departments who refer to the service.

**About you**

**Experience**
- Be a registered Band 7 clinician (adult trained) with an active NMC pin
- Happy to travel

**Desirable qualifications**
- Masters degree in a related field

**Desirable experience**:

- working in a digital start up
- working with remote patient monitoring

**Skills**:

- be able to read, write and speak English fluently;
- have solid technology acumen;
- have excellent written and verbal communication skills and well-developed interpersonal skills;
- be excellent at team working with the ability to work using their own initiative;
- be professional and patient-focussed;
- demonstrate the clinical knowledge and awareness of the complexities of remote patient monitoring;
- be accountable for their own actions in accordance with the Code of Professional Conduct;
- be able to work autonomously with mínimal direction;
- have well-developed political awareness, influencing, negotiating and conflict resolution skills;
- be able to motivate and inspire the clinical team to achieve quality patient outcomes;
- be confident using IT systems MS office (including word and excel)

This is an onsite role working five days a week in either Chesterfield and Royal Derby Hospital sites.

They may also be required to do some work at the following South Yorkshire sites:

- BHNFT - Barnsley Hospital NHS FT
- TRFT - The Rotherham Hospital NHS FT
- STHT -Sheffield Teaching Hospitals NHS FT
- DBTH -Doncaster and Bassetlaw Hospitals NHS FT
