Directorate: Children and Young People's Services

Permanent, 37 hours per week**.**

A wonderful opportunity has arisen to appoint two full time Assistant Education, Health and Care Coordinators to work in Rotherham’s Education, Health and Care Planning Team.** **The Education, Health and Care Planning Team performs Rotherham Borough Council’s statutory duties in relation to special educational needs and disability (SEND) statutory assessments and placements. This roles will comprise of hybrid working arrangements.

**The Service**:
Coordinates statutory assessment and maintains Education, Health and Care (EHC) plans for children and young people aged 0 - 25

Arranges placements and SEN provision including personalised budgets cited in EHC Plans

Monitors and reviews specialist placements

Ensures statutory annual reviews take place

Works closely with families, schools and a range of supporting professionals to support the work above and work towards positive outcomes for children and young people in Rotherham

As a member of this team the successful applicants will play a key role in ensuring that all processes are carried out in accordance with the Children and Families Act 2014 and SEND Code of Practice 2015.

We are looking for a flexible experienced workers who can:
Work sensitively and collaboratively with parents/carers, children and young people ensuring their views and wishes are considered throughout the process

Develop strong working relationships with services, agencies and providers to contribute to effective multi-agency working.

Ensuring records are kept up to date on specialist databases such as Capita.

Maintain a sound knowledge and awareness of professional business support practice, systems and technologies and use these in continuously improving the quality, productivity and effectiveness of the SEND service.
