Urgent Treatment Centre (Skegness, Boston and Louth) / Clinical Assessment Service

There will also be a £5,000 golden handshake bonus available, pro rata.
- Managing a wide and reaching case load, dealing with a wide range of presenting health care needs and undiagnosed problems across our 24/7 services
- Provide remote support to telephone consultations
- Triage a variety of calls and cases ranging from base appointments up to more complex cases requiring hospital admissions or referrals to specialists.
- Work towards completing 6 quality telephone consultations per hour or 4 base appointments per hour.

The overall aim of Lincolnshire Community Health Trust is to continue to provide an excellent Urgent Care Service which includes Urgent Care Streaming, Out of Hours, In-Patients and the Clinical Assessment Service, employing a combination of Urgent Care Salaried GPs, Specialty Doctors, Associate Specialists, Advanced Nurse Practitioners and Health Care Support Workers who can work across all the areas, at all base locations, with an element of individual choice in hours and location of work.

We also are becomingly increasingly involved in providing Primary care within the community, helping to support Practices in a variety of ways.

To provide, as part of a core team of Urgent & Primary Care Specialty Doctors, Salaried GPs, Practitioners, Trainee Practitioners, Nurses and other support staff, high quality in and out of hours medical services to the community of the Lincolnshire in line with local and national priorities.

To provide consultation and appropriate clinical management of patients accessing Urgent Care services in Lincolnshire, via NHS 111, 999 or by self-presenting at Urgent Care centre.

Where clinically indicated to provide consultation and appropriate management for patients during OOH who are visited at home or in institutions.

If rostered for CAS shifts, to provide efficient and clinically appropriate management by telephone triage consultations of patients contacting the service. Support of the CAS is also required at times of high demand for all Clinicians working within Urgent Care, including Clinicians that are rostered for other duties.

To develop and encourage positive working relationships with local GP Practices, housing, social work, voluntary services, drug and alcohol, mental health, dental, secondary care and other appropriate services.

To keep contemporaneous, accurate and legible records of all patients encounters and contribute to electronic data recording and audit as directed by LCHS (significant Event analysis will be undertaken in conjunction with the Medical Lead for Urgent Care and the Medical Director of LCHS)

To pursue continuing professional and personal development and undertake Appraisal and Revalidation as required by LCHS.

To participate in audit and other clinical governance processes.

To participate in the supervision, education and mentoring of students, trainees and other professionals as may be required by LCHS.

To assist in the facilitation of and development strategy in the continuing education and upgrading skills of medical and nursing staff.

To demonstrate a willingness to undertake additional professional responsibilities at local, regional and national levels.

The post holder may be requested to perform exceptional irregular commitments outside normally rostered duties as are essential for the continuity of care and in occasional emergencies and unforeseen circumstances.
