**Role Overview**:
The coordinator will serve as the primary liaison between health and social care professionals, families, and internal teams, ensuring a smooth referral and placement process. They are responsible for managing communications, conducting detailed interviews, and organizing all information required for a seamless transition into supported living homes or independent residential settings.
**Key Responsibilities**:
- **Referral Handling**
- Conduct thorough interviews with social workers, commissioners, and other stakeholders to gather essential background details for each referral.
- **Liaison**
- Maintain close collaboration with healthcare professionals, families, and internal departments to validate and confirm referral details, ensuring proper communication and understanding.
- **Process Management**
- Manage the referral process following established procedures and guidelines, ensuring each step is coordinated efficiently across all internal and external touchpoints.
- **Collaboration**
- Work alongside Operations teams, such as the Regional General Manager, Operations Manager, and Business Development Manager, to assess available homes for referrals and anticipate future vacancies.
- **Communication**
- Provide the Quality team with comprehensive, accurate background information for decision-making, ensuring all relevant details are shared promptly.
- **Documentation**
- Maintain up-to-date, accurate digital records of all referral data in dedicated SharePoint folders and keep tracking systems current.
- **Follow-up**
- Engage with referrers post-placement decisions to provide feedback on placement outcomes and reasons for acceptance or decline.
- **Guidance**
- Offer advice and support to Customer Service Advisors at the outsourced contact centre.
- **Transition Facilitation**
- Help coordinate pre-move arrangements for individuals transitioning into their new homes to ensure a smooth handover process.
**Role Requirements**:
- **Sector Knowledge**: Prior experience with referrals and placements within the adult social care sector, particularly related to learning disabilities, is essential.
- **Communication Skills**: Strong communication and interpersonal skills are required to liaise with multiple stakeholders, including social workers, healthcare professionals, families, and internal teams.
- **Task Management**: The ability to manage and prioritize multiple tasks in a fast-paced environment is critical to ensure smooth operation and efficient referral handling.
- **Technical Skills**: Proficiency in Microsoft Office 365, especially SharePoint or other CRM systems, for tracking and filing referral information is necessary.
- **Preferred Experience**: Previous experience in a customer service or coordination role is advantageous.
