We are currently looking for motivated and enthusiastic Band 6 Registered Mental Health Nurse, Social Worker or Occupational Therapist to join the Urgent Care & Assessment Team (UCAT) Home Treatment Team, formally (CRHTT) based at St Mary's Hospital, Kettering. The team operates over 7 days a week, 365 day a year to provide urgent assessments and home treatment to those experiencing a deterioration in their mental health.

The multi-professional team aims to work in coproduction with individuals and their carers to support them by identifying, assessing and planning their care; through early intervention and prevention work, the team will support individuals where possible to avert a crisis.

- Experienced clinicians with excellent assessment, dynamic risk assessment/management and communication skills.
- Proactive individuals with a passion for a recovery based approach, promoting social inclusion and enhancing an individual's wellbeing whilst working in coproduction.

NHFT is an integrated primary care and mental health Trust, providing physical, mental health and specialty services in both hospital settings and out in the community. Because we put the person at the centre of all we do, we focus on delivering care that is as easy to access as possible. This means many of our services can be provided at home, work or in schools. We also provide health services to various prisons and detention centres in Bedfordshire and Cambridgeshire.
- Triage of incoming referrals
- Arranging face to face assessments
- Gate keeping of admissions to hospital or referring on to the most appropriate service.
- Management of the daily shift with the delegation of daily duties to the team.
- Working collaboratively with service users, their carer’s and colleagues to positively manage treatment and risk in the community, whilst enabling and empowering an individuals’ recovery journey.
- Provision of supervision, education and training where appropriate.
- Undertaking very urgent front line assessments from GPs and other referrers
- Making clinical decisions for appropriate care planning and/or onward referrals.
- Coproduction of robust safe home treatment plans;
- Providing direct patient care to people in their home who are acutely mentally ill.
- To take a lead role in the the formulation, development, implementation and review of individual treatment plans.
