**Job Title**

Primary Care Network Digital Transformation Lead

The PCN is made up of three small practices all within 1.6 miles of each other in the North Oxfordshire town of Banbury. This role is for 37.5 hours per week and will require working at all three practices from time to time.

**Objective of the role**
- Increase efficiency and reduce duplication
- Provide easy to understand data packs to support decision-making that includes but is not limited to demand, capacity, utilisation data, staff and patient experience
- Build staff skills and capabilities
- Enhance patient access whilst protecting the PCN workforce
- Develop the digital maturity of the PCN
- Optimise the use of clinical systems
- Reduce variation between practices
- Support the achievement of IIF and Qof indicators

**Main Responsibilities**
- To develop and maintain an in-depth understanding of the PCN digital infrastructure
- To support the adoption of new technology and other initiatives to improve the care offer and increase PCN efficiency and productivity
- To proactively project manage all digital projects and also evaluate their effectiveness.
- To build relationships and facilitate collaboration between practices to support the delivery of care to patients.
- To lead the development and implementation of digital transformation within the PCN
- To ensure all staff receive training on the digital tools within the PCN
- To ensure the induction of all new members of staff receive training and resources on tools available
- To remain up to date with the national and local PCN guidance.
- To use data to understand demand, capacity and activity and then present that information to the PCN to enable improvements
- Support Population Health Management to enable improvements in care quality, patient access and PCN operational efficiency.

**Communication, relationship building and presentation skills**
- To work in partnership with the PCN leadership and management team
- To identify and develop relationships with practice staff, private providers, the local primary care team, neighbouring PCNs and other key partners to enable good working relationships and promote and share best practice
- To deliver technical information in a non-technical and engaging way to a range of stakeholders

**The first 90 days**
- Work closely with the PCN leadership and management team to understand the overall PCN strategy, which will underpin the digital infrastructure required.
- Conduct a strengths, weaknesses, opportunities and threats (SWOT) analysis of the current digital infrastructure of the PCN. This analysis will involve speaking with members of the PCN team.
- Review what subscriptions are in place and compile their due dates for renewal and cost to the PCN.
- Create a framework which encompasses fundamental principles to align and focus the PCN digital infrastructure, which may incorporate but not be limited to:

- Value for money
- Ease of implementation
- Level of interoperability
- Key performance indications
- Overall outcome
- Develop and present a finalised digital transformation strategy and implementation plan.

**Essential Experience**
- Experience in managing IT systems
- Experience in delivering programmes of IT projects through the entire project lifecycle.
- Experience in delivering IT projects across multiples sites
- Accustomed to working in a busy environment
- Ability to work on own initiative and organise workload accordingly

**Desirable experience**
- Education to degree level or equivalent professional experience
- Business Analysis: at least one year
- Data management: at least one year
- A positive, proactive and forward thinking attitude
