Crewe Mental Health Service are seeking an enthusiastic community mental health nurse or alternative mental health practitioners to join our busy team.
Do you have a passion for supporting people within their own home to live as independently as possible? You will be joining a busy community mental health team who provides support to adults with a serious mental health condition such as Bi Polar Affective Disorder, Schizophrenia, Severe Depression to maintain their quality of life and remain well in the community. You will also be working to support Adults with a dementia, along the diagnostic and treatment pathway. This also may include completing reviews and assessments in our locality nursing and residential homes.
We are ideally looking for a practitioner with experience of both functional and organic mental ill health. However, we would consider a nurse with experience of one of these areas as training and support will be given to develop your skills and experience.

The post holder will work autonomously within a multi-disciplinary framework, undertaking assessments and coordinating the care of patients with both functional and organic mental illness.
The post holder will maintain a caseload which will involve ongoing assessment: Formulation of care planning, risk assessments. Completing reviews and offering interventions to help improve wellbeing/outcomes for the individual and their families, signposting to other agencies as necessary. Excellent communication, report writing, and assessment skills are essential for this post. Applicants should have sound working knowledge of mental health disorders and the Mental Health Act.

Access to transport for work purposes is essential.
Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all-age disability care, as well as the provision of three GP surgeries in Cheshire.
