Home Care Practitioner
Part time - 24 hours per week
An opportunity has arisen for a Band 2 Home Care Practitioner to work as a member of Keswick and Solway Integrated Care Community, Rehabilitation Team. The post holder will be required to work across the ICC an area of outstanding natural beauty encompassing the Lake District National Park and Solway Plain.
We are looking for a motivated and caring individual, preferably with some community experience, but more importantly someone who is enthusiastic about rehabilitation and providing person centred care to people and who enjoys working in a team.
The ability to work over 7 days is a requirement of the post. The hours of work will be on a rota covering a shift pattern between the hours of 8 a.m. and 10 p.m.
ICCs aim to address the challenges of an aging population and increasing demand on health and care services by providing more out of hospital care and empowering people to take control of their health and wellbeing. This will require new ways of working to focus on preventing ill health and ensure services are sustainable for the future. These new ways of working will follow the same approach across all eight ICCs with flexibility to tailor services to the needs of local people.
The Community Rehab Team is a friendly multidisciplinary team, with staff who work together to facilitate early discharge from hospital or prevent unnecessary admission.
The team has close working relationships with all the local health and social care teams, and we pride ourselves on our flexibility and ability to respond to meet the needs of our patients.
As a Home Care Practitioner within the ICC, you will work with patients in their own homes to provide short-term support, aiming to maximise independence and increase confidence and quality of life.
The ability to travel independently across a wide rural area is essential

**Disclosure and Barring Service**:

**Standard Check**: £18 + Administration cost of £3.85 = £21.85. Deducted at £6.85 from 1st month salary and £5 deducted from salary over following 3 months.
**Enhanced Check**: £38 + administration cost of £3.90 = £41.90. Deducted at £11.90 from 1st month salary and £10 deducted from salary over following 3 months.
