The post holder will be an experienced Advanced Clinical Practitioner who, acting within their professional boundaries will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. Demonstrate safe, clinical decision-making and expert care for patients within the general practice. Perform as an autonomous practitioner working independently and with other members of the health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiating direct referrals as appropriate. Manage daily surgeries comprising face to face consultations, home visits and telephone consultations.
Manage results and workflow in line with practice policy and procedures. Work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures. Build effective working relationships with other health care professionals and will work within the team of General Practitioners and Nurses to deliver excellent primary health care services within the practice.
