We are looking for passionate and motivated registered health care professionals to join our dynamic urgent & emergency care team at Northampton General Hospital as Streaming Practitioners.
The Streaming Practitioner role is central in managing the flow of patients into and through the Emergency Department and Urgent Treatment Centre and ensuring that patient is seen by the most appropriate part of the urgent care service to meet their healthcare needs.
Working within the streaming hub which is the front door to the Emergency department, the streaming practitioner is responsible for autonomously assessing and directing the patients to the most appropriate pathway for their clinical requirements as well as providing hands on care to a range of patients presenting including taking baseline observations, physical health assessments, administering analgesia or drugs as prescribed by a practitioner or on PGD, requesting investigations, taking blood samples and or providing treatments as directed by the practitioners.
Where possible the streaming practitioner will identify patients suitable to be directed direct to specialist services as well as autonomously assessing and discharging non-acute presentations which require primary care rather than urgent care provision.

Main Responsibilities
Supporting Clinical Excellence
Undertake face to face autonomous assessment and streaming of patients with undifferentiated and undiagnosed presentations attending ED to the most appropriate area in the service to meet their health needs. Recognising red flag signs and symptoms suggestive of the acutely unwell patients to ensuring they are prioritised appropriately.
Carry out physical clinical assessment when required to support the streaming process and ensure patient safety. Carry out investigations which will support clinical decision-making when reviewed by the clinicians.
Within the UTC, triage the direct 111 attendances within 15 mins post registration using the Manchester Triage system to identify patients to manage as a priority whether in the UTC or referring to ED if found to be acutely unwell.
Identifying patients with non-urgent healthcare issues which do not need medical assessment or treatment and effecting a safe discharge back to community services.
Identifying patents who fulfil the criteria of agreed referral pathways direct to speciality and making those referrals.
Identifying when needed and administering medicines under PGD such as analgesia, antihistamines, nebulisers etc. to ensure that the patient comfort and safety during their attendance and enhancing patient experience.
Liaising with the clinical teams within ED and UTC to escalate concerns or agree treatments for patients as necessary.
Liaise between ED/UTC/SDEC and OOH to facilitate the flow of patients and ensure safe appropriate referrals.
Professional Responsibilities
Adhere to the NMC or HCPC Codes of Professional Conduct and always work within their own scope of professional practice.
Ensure professional practice is in accordance with the NMC, HCPC and the Trust’s own policies, procedures, and guidelines.
Maintain a professional manner at all time and acts a positive role model for all members of the Urgent Care team.
Ensure accurate contemporaneous record keeping and documentation in line with Trust and NMC or HCPC policy and guidance.
Attend annual appraisals with line manager and maintaining professional registration.
General Responsibilities
Participate in and support service improvement efforts by being flexible and open to changes which better meet service needs.
Participate in clinical audit to identify best practice and areas for improvement.
Have a zero-tolerance approach to bullying and harassment of all kinds and promote equality and diversity in the workplace. Lead by example in this remit.
