**About the role**

We have an exciting new opportunity for a Dementia Support Worker to join our Community Dementia Support team in Essex. We enable people affected by dementia to receive support and information, including local support groups, Lasting Power of Attorney, and practical advice on living well with dementia. This service provides responsive and individualised information, signposting and referral service to people affected by dementia.

You will need to be flexible in your work to ensure that we can provide support across the area. It is essential that applicants can travel independently within the Essex area.
- Assist people with dementia and their carers in identifying their needs, providing information and guidance to help maintain their independence, improving their sense of well-being, and putting them in more control of their lives.
- Be part of a highly motivated and supportive team, sharing responsibility of some group activities.
- Build relationships with a range of local contacts, networking with health and care professionals, marketing the Dementia Connect Service and providing dementia support.
- Develop networks and pathways with partner organisations who work in the field of dementia, ensuring seamless and consistent support and empowering individuals affected by dementia to make informed choices.
- Provide support face-to-face, by telephone and online.
- Supporting and empowering people and their families with expert guidance and advice

**About you**
- Possess NVQ level 3 (or equivalent) or have demonstrable experience
- Be empathetic and non-judgmental in your approach with a commitment to equal opportunity.
- Understand client confidentiality and how this is applied when representing client needs
- Able to travel independently around South Essex, occasionally be required to travel to other areas in Essex and Herts where the need arises
- Have knowledge and experience of Adult and Children Safeguarding, Information Governance policies and procedures and statutory laws such as the Mental Capacity Act**:

- Have some experience of working with a wide variety of relevant agencies**
- **Desirable but not essential**

- Be passionate about making a positive difference in the lives of those affected by dementia.
- Demonstrate optimism, be engaging, and be a pragmatic individual.
- Excellent interpersonal skills, being approachable and friendly.
- Excellent communication skills, both verbal and written.
- Great organisational skills to manage your caseload of clients effectively.
- Excellent IT skills and ability to use Microsoft packages confidently.
- Be able to work collaboratively with other team members if/when required.

**This role will require an enhanced DBS (Disclosure and Barring Scheme) check.
