Our Social Care Access Service (SCAS) offers a dedicated and professional service which is responsive to the needs of the citizens of Hertfordshire. We seek to promote independence and wellbeing to citizens which celebrate use of community solutions. As one of our Community Care Officers, you’ll ensure that we continue to deliver this incredibly important work.

Working as part of our Targeted Intervention Teams our Community Care Officers support and help citizens to remain at living at home following an injury, poor health, disability or after a spell in hospital. Under the new model of Social Care for Hertfordshire, Connected Lives, you’ll undertake an assessment to gather facts and information from the person. Upon completion, you’ll create a enablement care package dependent on their needs and the support they require, this could include adjustments such as minor adaptations to the home or arranging for support from friends, families or professionals, all of which has the key aim at ensuring people remain independent & connects people with their communities.

Whatever the situation, you’ll have the support of a well-established, experienced team that is committed to making a difference to the people who rely on this service.

We have a number of full-time positions available with immediate starts available. We are looking for professionals with creativity and enthusiasm to transition our services back to Hertfordshire County Council as part of their exciting new ‘Gateway’.

**What you’ll need to do the role**:
You’ll naturally be empathetic, caring and compassionate in your approach. Ideally, you’ll have experience of working within a customer focused environment, a good understanding of the Care Act and have previous experience of carrying out assessments/support planning in a social care setting. But more importantly you will have exceptional customer service skills, particularly with the ability to build rapport with people in order to gather important information.

You will be highly organised and hard-working and have a true passion and enthusiasm for providing a high level of care at all times.

A drivers license is required as is access to your own vehicle.
