Digital Clinical Hub Care Coordinator

Duties to include; To assist the PCN in developing processes that improve the way that clinical correspondence is coded, filed, and processed to benefit both practices and patients. Support Practices with coding and filing all appropriate clinical correspondence who are aligned with the Digital Clinical Hub. To aid in the direct care of patients by requesting investigations, arranging appointments, and improving the readability and understandability of the electronic health record. Utilise current models of practice such as workflows and templates for accurate recording of clinical information and help develop appropriate reporting systems.
Accurately coding records using standardised SNOMED coding to improve data quality. Engage in audit and analytics work to aid the digital clinical hub and PCN practices. Assist member practices in digitising workflows. Be part of a team providing this support, by working with colleagues to share skills, expertise, and knowledge to improve processes for all of our GP practices.
Provide administrative support to GP practices that request additional assistance regarding clinical correspondence or administrative requests, as agreed with the PCN. Attend any relevant identified training that will assist in the development of the Digital Clinical Hub and your own personal development. Build and develop relationships with team colleagues, wider PCN colleagues and with practices across the PCN. Undertake other duties as may be reasonably required within the general terms of the job description.
