The patient placements or care packages are routinely highly complex and some patients can exhibit extreme and unpredictable levels of challenging behaviours. The post holder will be responsible for providing clinical advice and guidance to colleagues relating to the patients individual care package and planning for future care provision. Work with partners to safeguard smooth and safe transition of care packages ensuring all agencies are involved and the individual is at the heart of their care pathway. A resource to support other agencies and service providers with social care knowledge and advice of alternative care placements and treatment options to ensure all placements provide high quality services with proven positive healthcare outcomes and value for money.

Review and re-assess care packages to ensure patient placements are suitable, provide positive health and wellbeing outcomes. Meet with patients and carers within services and ensure clear understanding of case management and complex programmes of care on a continuous basis, discuss potential transfers to alternative provision with patients when necessary. Ensuring the individual patient is fully consulted and involved in their care pathway. Attend patients reviews and pre-discharge meetings to ascertain appropriateness of placements Request patient review meetings when concerns are raised by patient, their carer or by ICBs to discuss quality of care issues and figure care pathways.

Identify alternative service providers and ensure effective service provision by inspecting services countrywide, in relation to Key Performance Indicators. Support financial management of the budget and provide guidance and recommendation on the funding of individual care packages, this will include highly contentious funding decisions when multiple agencies are involved in the individual care planning. Partnership and Cross Boundary working To work collaboratively with all stakeholders in the local area to manage relationships with ICB commissioned providers that provide the referral and aftercare components of the individuals care pathway. In relation to Section 117 work, to work in collaboration with the criminal justice system - providing expertise and advice on the interface of individuals who are in receipt of custodial care and who have complex healthcare needs with associated risks to themselves or the public.

This is often in relation to people who have committed a criminal offence and who are of high interest to the media and public. When required provide advice and guidance to non-clinical staff within the ICB. Project development and implementation Highlight areas of concern including Safeguarding and the identification of deficits in service provision to the relevant senior officers within health or social care and work alongside professionals on recovery plans or relevant investigations. Ensure any new systems are supported by strong governance that safeguarding remains paramount as a principle within the approach taken.

To work with providers to ensure they deliver relevant national care standards Ensure all information pertinent to the individuals care packages is up to date to support financial forecasting.
