**Care Coordinator**

**Location**: Sutton, SM2 5AE

**Salary**: £24,000 -£30,000 per annum

Our client was founded in 2014, since then they have established a very high quality reputation providing Domiciliary Home Care as well as Live-In Care.

The company is registered with the Care Quality Commission which is the government body set up to regulate Home Care provision within the UK.

They pride themselves on enhancing the quality of life for their customers, enabling them to lead as independent a life as possible 24 hours a day 7 days a week in the comfort of their own home. They tailor each of their home care packages to the needs ofthe individual.

**Role Requirements**:

- Ideally some Care Coordinating experience
- At least 6 months experience working as a Care Coordinator
- A caring attitude and a willingness to make a difference
- Reliability, flexibility, hard work and honesty
- Great communication skills
- Great team work skills
- The ability to remain calm and composed under pressure

**What Would Your Responsibilities be?**

As a Care Coordinator, you will be working in the Sutton office Monday to Friday.

**Duties include, but may not be limited to**:

- Allocating carers to their assigned customers
- Completing rotas and ensuring are sent each week
- Maintaining excellent communication with your team and customers
- Supporting and developing care staff
- Managing the on call service on a rostered basis
- Liaising with Customers, Staff, Social Services, GPs, Pharmacists and CCG
- Working in a team to ensure high customer service levels

All staff wear full PPE when with customers, the company also provide all their customers with hand gel and masks. Office protocols are to gel on the way in and out of the office and sanitise communal areas regularly.
