Trafford Local Care Organisation are a public sector organisation bringing together NHS Community Health and Social Care services in Trafford, driven by the aim of improving the health of local people by working across organisational boundaries. Our community healthcare teams are employed by Manchester University NHS Foundation Trust and deployed to us, linking closely with colleagues in Manchester Local Care Organisation. Our teams work as one and together we will make a massive difference to the health of people in Trafford.
They will develop skills and competencies to enable them to have ongoing responsibility for the assessment of children and families with complex or single-care needs.
They will also develop skills and competencies to ensure they can implement and evaluate a programme of care in partnership with other agencies, ensuring the delivery of prescribed high-quality care through effective leadership of the nursing team.
During supervised practice they will have delegated responsibility for a selected generic caseload and, under supervision, will provide assessment of care needs, the development, implementation and evaluation of programmes of care for a designated client population.
